Diabetes

Did you know the prevalence of Diabetes is 20% higher in South Texas than the ENTIRE state?
More than 1/4 of these people have uncontrolled Diabetes which can lead to multiple complications including (but not limited to): heart attack, stroke, diabetic wounds, neuropathy, kidney problems among others.
Many of these individuals end up hospitalized for complications that could have been prevented or managed before they became too severe.
Kare Elevated Nurses are able to assist you with important monitoring and care including diabetic foot assessment, diabetic foot care, education on disease management, medication management, assist with weekly insulin injections as prescribed by your doctor and monitoring for side effects of your current medications as well as assisting with advocating for you to ensure you receive the best care possible within the healthcare system.
High Blood Pressure
(Hypertension)

High blood pressure is commonly called a "silent killer" as typically there are no symptoms.
This is why your doctor recommends regular blood pressure checks. Blood pressure is the strain blood flow places against arteries as your heart pumps blood throughout your body. Blood pressure rises and falls throughout the day, but health problems can arise when it remains high for an extended time period. When you have high blood pressure, your heart has to pump harder to circulate blood throughout your body which can increase your risk of stroke or heart attack.

There are many ways that you can decrease your blood pressure including medications prescribed by your doctor and various lifestyle changes.
We will work closely with you to help lower your blood pressure and improve your overall health through regular blood pressure checks (as often as you like), education and can assist with medication management including prefilling medication boxes to ensure you take all medications your physician has prescribed to you.

Advocacy

Many people are not familiar with the medical terminology that doctors and other health care professionals use. Kare Elevated nurses can break down complicated information regarding diagnosis, medication, and treatment plans.
Allow up to give you a voice in situations or setting where you could possibly be ignored or disregarded.

We can accompany you to Dr Appointments, report back to family members if you would like, and assist you with effectively managing your care in a discreet and professional manner.

Kare Elevated Nurses can be utilized in many different ways to make your healthcare experience unique to you.
We do not have to be ordered by your doctor, rather all we need is for you to request us. We are able to assist with many different areas including (but not limited to)​
